This report sets out an integrated and comprehensive, evidence-based approach to supporting people with dementia living at home during the moderate to severe stages of the illness.

This report identifies the barriers that limit the wider implementation of personalisation and provides key recommendations that will enable the majority of people with dementia and their carers to personalise their care and support.

This report is the outcome of a study investigating the views and experiences of people with dementia, carers and social work care managers across Scotland.
